Your service coordinator
Your service coordinator helps you understand the rules and how things work. They help you speak up for yourself or your family member. They help you find and get services.
Behind them is a team: clinical staff, intake, finance and more, all working to get the right services to you.
Your plan
You ask for services through your plan. For people 3 and older, the plan is called an Individual Program Plan (IPP). For babies and toddlers, it is an Individualized Family Service Plan (IFSP). You and your planning team write it together.
The Self-Determination Program
The Self-Determination Program is another way to get your services, with more choice and control over who provides them. Learn about the Self-Determination Program (on the current WRC website)
