For service providers

We work with hundreds of service providers, also called vendors. Together, we help people with developmental disabilities live safely and with dignity in our community.

Becoming a provider: what’s changed

  1. Use the Provider Directory

Since March 1, 2026, every new vendorization goes through the Provider Directory.

  2. Have a business address in our area

Your physical business address must be in our service area. We do not accept PO boxes.

  3. Send a letter of intent and the disclosure form

The Applicant/Vendor Disclosure Statement (DS 1891) is in English and Spanish.

Everything for providers

Design noteYour Vendor/Provider menu has 24 links. Here they sit in six groups on one page, so any of them is two clicks from the homepage. Families never have to scroll past provider content to find their own.
